When you arrive, check in with the receptionist and they will give you a few forms to fill out. For your convenience, please click on the directions and maps to our office.

Please come 15 minutes before your appointment to fill out some paperwork. Dr. Berman, unlike most physicians, respects your time and will make every effort to see you exactly at your scheduled appointment time, so please make every effort to arrive on time.

Breast Reduction Consults
Breast reductions are frequently covered by insurance companies. I will have to obtain some information from you and will also have to submit photographs to the insurance company. It usually takes about 6 weeks for you to hear from the insurance company. We will get a copy of the letter they send to you. Remember to bring a referral from your primary care physician, if your insurance company requires this for any consults which are not purely cosmetic.
